CRBA Required Documents: The Checklist That Saves a Second Trip

Most times a CRBA drags on longer than it should, the reason isn't red tape, it's one missing document at the first appointment. The core list is the same for everyone, but a few additions depend on the family's situation, and it's worth knowing them in advance.

The base every family needs

• The baby's full Israeli birth certificate, original plus two photocopies
• The US-citizen parent's passport ⁦(original)⁩
• Proof of the US-citizen parent's physical presence in the US before the birth
• A compliant passport photo for the baby

If the parents are married

Add a marriage certificate. If either parent had a prior divorce or a deceased spouse, add the relevant divorce decree or death certificate too, to document that the current marriage is valid.

If one parent isn't a US citizen

The foreign parent completes Form DS-5507, detailing periods of time spent in the US and biographical information. It doesn't make them part of the application itself, but without the form, the filing is considered incomplete.

If one parent can't attend the appointment

The absent parent completes Form DS-3053, a Statement of Consent, which must be an original, signed, dated, notarized document, along with a copy of the ID shown to the notary. Without the original, not a photocopy, the application isn't accepted.

Extra evidence that helps, never hurts

The mother's prenatal records, hospital documentation, and proof that both parents were in the same place around the time of conception aren't required in every case, but they strengthen an application that isn't clear-cut.
